Female infertility: causes, signs, tests and treatment – Dr. Chanchal Sharma

How does pregnancy take place?

Have you ever thought about how pregnancy takes place? And what are the things that obstruct it? It takes place when an egg from a woman is penetrated by sperm from a male. There is a whole process from ovulation of the eggs and their fertilization to the delivery of the baby.

A woman has ovaries that produce eggs. When an ovary releases an egg, it is collected by a fallopian tube. If the egg is fertilized by male sperm, it sticks into the endometrium (the inner lining of the uterus) and grows there. Problems with the ovaries, fallopian tubes and the uterus can cause infertility.

What is female infertility?

If a woman has been trying to become pregnant with frequent unprotected sex, not using any contraceptives, for more than one year (six months if the woman is over 35), it can indicate infertility.

What are the causes of female infertility?

If we talk about the infertility causes, there are several. Some of them are mentioned below:

  • Ovulation disorders. Ovulation disorders can include polycystic ovary syndrome (PCOS), premature ovarian failure & menopause, and hypothalamic amenorrhea. Ovulation disorders can impair the production and the release of the eggs.
  • Fallopian tube damage or blockage. The fallopian tube plays a crucial role in bringing an egg from the ovary to the uterus. It is usually the fallopian tube where the egg is fertilized. The egg can remain stuck in the fallopian tube due to a blockage or damage to it. The blockage can also block the path of sperm into the tube.
  • Abnormalities of the cervix and the uterus. The abnormalities of the cervix and the uterus, including the polyps in the uterus and the shape of the uterus, may also cause infertility.
  • Endometriosis. The endometrium has tissues that break and bleed during menstruation. The endometrial-like tissues can build up outside the uterus and act the same as the tissues of the endometrium do. This condition is called endometriosis. It can damage or block the fallopian tubes and the ovaries.
  • Pelvic adhesions. Pelvic adhesions are actually the bands of scar tissues that can cause organs to stick together. They can take place in the ovaries, the fallopian tubes, the uterus and other places in the pelvic cavity.
  • Cancers.  Certain types of cancers, such as uterine (endometrial) cancer, ovarian cancer and cervical cancer, can cause infertility. Even the treatment of cancer can have a bad effect on fertility.
  • Early menopause. It is said that smoking, consuming alcohol, certain treatments, and medicines may cause early menopause. However, more research is needed on what exactly causes premature menopause. Early menopause is when menopause happens before the age of 40.

What are the symptoms of female infertility

If we talk about the infertility symptoms, infertility is the main symptom. A woman may also have other symptoms such as irregular or missing periods, pelvic pain or vaginal discharge. But, these symptoms may also be shown in other issues, not only in infertility.

How can female infertility be diagnosed?

There are a few tests that the doctor can recommend for diagnosing the cause of infertility in a woman.

  • Blood test. A blood test can be done to find out the levels of hormones, such as thyroids hormones and others.
  • Pelvic exam. A pelvic examination only runs for a few minutes. The doctor checks the patient’s uterus, rectum, cervix, vulva, vagina, ovaries, rectum and pelvis for any abnormalities. The doctor can also perform a pap test to screen for cervical cancer.
  • Transvaginal ultrasound. With the help of transvaginal ultrasound, the doctor examines the reproductive organs of a woman. This ultrasound is done by inserting an ultrasound wand into the uterus through the vagina.
  • Laparoscopy. It is done with the help of a thin tube (a laparoscope) that has a light and a camera. The surgeon makes a tiny cut in the abdomen of the patient in order to insert the laparoscope and examine the reproductive organs inside.
  • Hysterosalpingogram (HSG). The doctor tries to detect blockages by observing the flow of a special dye sprayed into the uterus through the vagina. The flow is watched through x-ray imaging. If there are blockages in the fallopian tube, the test may find out.
  • Hysteroscopy. The doctor examines the uterus by inserting a thin, lighted tube through the vagina.
  • A sonohysterosalpingography. In this test, the doctor uses sterile air, saline fluid and transvaginal ultrasound to find out if any blockages are present in the fallopian tubes. The doctor sprays saline fluid with sterile air into the uterus through the vagina. And the flow is monitored by a transvaginal ultrasound.
