What is Azoospermia ?

Azoospermia means : semen without sperm. When a man ejaculates after orgasm, no sperm is found in his ejaculate (semen). This condition causes male infertility. How common is it? It is said that around 1% of all men have azoospermia; and around 10% to 15% of infertile males are diagnosed with azoospermia.

Types of azoospermia

It is said that there are two types of azoospermia.
1. Non-obstructive azoospermia. In this type of azoospermia, a man has poor or no sperm because of the defects in the function or structure of the testicles, or other health conditions.
2. Obstructive azoospermia. In this type of azoospermia, a man’s ejaculate has no sperm because of some obstruction. It could have due to a missing connection or a blockage in vas deferens, epididymis, or in some other place, including the reproductive tract.

Symptoms of Azoospermia

Azoospermia is a lack of live sperm in a man’s ejaculate (semen). Apart from this, it has no particular symptoms.

If the male partner has azoospermia, the couple trying to conceive will have infertility problems. If the male partner has the nil sperm count, the female partner will not get pregnant. If a couple does not achieve pregnancy in spite of unprotected sex for one year, it indicates infertility.

In this case, the couple must seek guidance from a doctor. The doctor will ask for information about the history of their health issues, and will do tests, if needed, to diagnose the cause of infertility. It is also important for the doctor to find out which partner is infertile so that the necessary treatment can be given to the infertile partner.

There are various factors that could cause infertility. Azoospermia is one of them. However, it has no particular symptoms, the following signs may indicate that you are at risk of it.

  • Pain during urinating
  • Dry orgasm (little or no orgasm)
  • Foggy urine after sex
  • Swollen testicles
  • Pelvic pain
  • Undescended or small testicles
  • Delayed puberty
  • Problems with ejaculation or erection
  • Decreased hair growth
  • Enlarged breasts
  • Low sex drive

Causes of Azoospermia

The causes of azoospermia can be obstructive or non-obstructive type. Obstructions that cause this condition commonly take place in the vas deferens, ejaculatory duct or epididymis. Conditions that could cause blockages in these areas are:

  • Infections in these areas
  • Injury
  • Inflammation
  • Vasectomy (planned surgery of vas deferens to stop the flow of sperm; a contraceptive procedure)
  • Prior surgery in the pelvic area
  • Cyst formation
  • Cystic fibrosis gene mutation

Non-obstructive causes can include:

  • Retrograde ejaculation or other ejaculation problems
  • Endocrine disorders/hormonal imbalances, such as androgen resistance and hyperprolactinemia.
  • Testicular causes that may involve
  • Cyptorchidism (testicles have not descended into the scrotum)
  • Anorchia (testicles not present)
  • Testicular torsion
  • Anti-sperm medicines (that harm the production of sperm)
  • Tumors
  • Diseases, such as kidney failure, diabetes or cirrhosis.
  • Radiation treatment
  • Cancers and their treatment
  • Varicocele (veins of the testicles become swollen)
  • Spermatogenic arrest (testicles produce sperm cells that are not fully mature)

How is it Diagnosed?

Azoospermia is diagnosed through semen analysis. The doctor can ask you the following questions to collect your medical history.

  • Illnesses in the childhood
  • Any prior fertility success
  • Surgery or any injury in the pelvic regions
  • History of reproductive or urinary tract infections
  • Your past and current medicines
  • Exposure to chemotherapy or radiation
  • Any prior sexually transmitted diseases
  • Substance abuse
  • Family history of learning disabilities, birth defects, cystic fibrosis or reproductive failure
