What is Hydrosalpinx? How does it cause infertility?
Hydrosalpinx meaning
The condition in which a fallopian tube is filled with water or fluid and becomes blocked is called Hydrosalpinx. The fallopian tubes are two narrow tubes in the female reproductive structure that collect eggs from the ovaries and transport them to the uterus
Hydrosalpinx does not affect the production of the egg in the ovaries. It causes blockage in the fallopian tube because of which it becomes impossible for an egg and sperm to meet in the tube and conception fails.
Mostly, sperm penetrates an egg while it is in the fallopian tube. After the fertilization process, the egg takes place in the uterus for its further growth.

How does it cause infertility?
Hydrosalpinx can cause infertility as it prevents the meeting of the egg and sperm due to the blockage created by the collected fluid. And secondly, it also obstructs the path of the egg due to which the egg remains stuck in the tube and does not reach the uterus.
Finger-like projections at the end of the fallopian tubes that collect eggs from the ovaries, called fimbriae, may close up due to some infection and fluid gets trapped in them.
Symptoms of Hydrosalpinx
Hydrosalpinx does not give any symptoms. This condition is asymptomatic. But some patients complain of pain in the abdomen or pelvic area. Many patients find out that they have Hydrosalpinx when they visit a doctor for having infertility problems.

Causes of Hydrosalpinx
Hydrosalpinx may occur due to an infection in the fallopian tube, often long and untreated. Infection may cause the fimbriae to close up, storage of fluid taking place in them. There could be other reasons as well, like:
- Endometriosis
- Fibroids
- Scarred appendicitis
- Pelvic inflammatory disease (PID)
- Sexually transmitted diseases (STDs)
- Abdominal surgery
- Injury
Diagnosis of Hydrosalpinx
The doctor will diagnose Hydrosalpinx in the following ways:
- A sonohysterosalpingography: In order to detect hydrosalpinx, the doctor will observe the flow of sterile air with saline fluid into the fallopian tube. Saline fluid with sterile air is sprayed into the uterus via the vagina. And to detect the flow, the doctor does a transvaginal ultrasound.
- Hysterosalpingogram (HSG): Saline fluid with sterile air is used in this to detect the flow in the fallopian tube. But the flow is observed with x-ray imaging.
- Laparoscopy: This test is most reliable as the doctor can see inside the fallopian tubes through a laparoscope. A laparoscope is a thin tube that has a light camera at the front. The surgeon enters it inside the uterus through a tiny hole made in the abdomen.
- An ultrasound: However not very reliable in this case, an ultrasound can still give some good hints about the increased size of the tube due to fluid in it.

Hydrosalpinx treatment
The most common treatment for Hydrosalpinx is surgery. The surgeon repairs the tube by removing the fluid from it through a surgical procedure. Or if needed, the tube is removed completely. But surgery always contains its risks like scar tissue, endometrial growths or adhesions.
If repairing of the tube goes well and there is a possibility of a natural pregnancy, the patient is left for a natural conception. But the patient is also kept under observation for an ectopic pregnancy or other issues.
An ectopic pregnancy takes place when the fertilized egg implants itself outside the uterus. It may remain stuck in the repaired fallopian tube and grow there. This condition is dangerous to the patient.
Another option is an IVF pregnancy. For an IVF pregnancy, first, the growth of the eggs in the ovaries is stimulated, and then the eggs are taken out from the ovaries. Fertilization of the egg is done in a laboratory and then it is placed in the uterus of the patient for its growth.
One must know that IVF is not always successful. And it also involves the risk of infection, scar tissue and multiple pregnancies.
